In the document released by the Indiana Department of Health and reviewed by NPR, Dr. Caitlin Bernard says she provided a medication abortion to a 10-year-old girl at Indiana University Health Medical Center in Indianapolis on June 30. That procedure uses pills to induce an abortion and involves a two-drug protocol approved for use up to 10 weeks of pregnancy.


Across the United States, the Centers for Disease Control and Prevention (CDC) indicates that nearly 3 million women have experienced rape-related pregnancy in their lifetimes, with an estimated one in nine girls under the age of 18 experiencing rape, according to the Rape, Abuse & Incest National Network (RAINN).

A detective testified Wednesday at an initial court appearance for the 27-year-old suspect that Columbus police learned about the girl's pregnancy through a referral by Franklin County Children Services that was made by her mother June 22, and that she had an abortion in Indianapolis on June 30, The Columbus Dispatch reported.


An Indianapolis physician who provides abortion services had told The Indianapolis Star that an abortion had been provided for such a child because the girl couldn't get the procedure in Ohio under a newly imposed state ban on abortions at the first detectable "fetal heartbeat." A judge lifted a stay on the ban after the U.S. Supreme Court's ruling overturning Roe v. Wade.


Court records don't specify whether or how the suspect knew the girl. The prosecutor's office declined to comment on the case, and the police department did not respond to a request for additional details. The Associated Press generally doesn't identify victims of sexual assault and, for now, is not naming the suspect to avoid inadvertently identifying the girl.


Dr. Caitlin Bernard, an obstetrician-gynecologist in Indianapolis, received a call last Monday, three days after the Court's ruling, from a child abuse doctor in Ohio who had a 10-year-old girl in their office who was six weeks and three days pregnant, and therefore ineligible to receive an abortion in the state, according to a report from The Indianapolis Star.


However, these so-called "fetal heartbeat" bans on abortion have faced criticism from medical experts, many of whom argue there is no heartbeat at six weeks. Rather, doctors can detect a flutter in the region where the heart will eventually form. Additionally, many women and girls do not know they are pregnant at that point.


A ban has not yet taken effect in Indiana, which meant that the girl was able to travel to Dr. Bernard for the procedure. Other information about the child's case was unknown, likely due to her age. Bernard told the Star that "it's hard to imagine that in just a few short weeks we will have no ability to provide that care."


Critics argued the girl's situation shows the devastating impact of the Supreme Court's decision, which will result in other children who are victims of rape being forced to carry their pregnancies to term.


A 10-year-old girl in Colombia recently gave birth via caesarian section, placing her among the youngest mothers in the world. Though the girl is now recovering, her case highlights the dangers of pregnancy before maturity, doctors say.


According to Good Morning America, the 10-year-old girl, a member of the Wayuu tribe in Manaure, Colombia, delivered a 5-pound (2.26 kilogram) baby girl. The age of the father is not known, but Colombian police reportedly cannot press charges as the tribe is under its own jurisdiction.


The greatest danger, however, is to the pelvic floor. Girls may start ovulating and menstruating as early as age 9, though the average is around 12 to 13. (Some studies suggest that the average age of first menstruation is dropping, but the data is not conclusive.) Just because a girl can get pregnant, though, doesn't mean she can safely deliver a baby. The pelvis does not fully widen until the late teens, meaning that young girls may not be able to push the baby through the birth canal.


The women and girls who do survive often develop fistulas, which are holes between the vaginal wall and the rectum or bladder. When the baby's head pushes down and gets stuck, it can cut portions of the mother's soft tissue between its skull and her pelvic bones. As a result, the tissue dies, and a hole forms. Feces and urine then leak through the hole and out of the vagina. Women with fistulas are often divorced and shunned. And young girls are at higher risk.


"The younger you are, the more trauma will occur, because the pelvic floor isn't developed enough," Thomas said. In that way, she said, the young Colombian girl was fortunate to have access to a hospital that could provide a caesarean section.


As growth tends to slow in girls once menstruation starts, a 10-year-old capable of getting pregnant is likely to be especially small, with a small pelvis, Wall said. And even if puberty onset is happening earlier (Wall isn't entirely convinced by the current data), pelvises are certainly not maturing any faster, he said. If puberty does occur earlier, that would put young girls at risk for dangerous pregnancies for a longer period of time.


But the story was questioned by some abortion opponents and fact-checkers, given its single source reliance on a Indianapolis obstetrician-gynecologist who said she had spoken to a "child abuse doctor" in Ohio, and its scant detail. The outlet defended its reporting. It drew scrutiny as well from the state attorney general, who said that exceptions to Ohio law would have allowed the girl to obtain an abortion in the state and who said he had not heard of a report being filed in the case.


According to her parents, Noella started saying she wasn't a boy when she was just two years old. At four, she went to therapy and declared to the doctor that she was a girl. While her parents believe she's too young to medically transition, she socially transitioned at four, and at seven legally changed her name. That's also when she started her modeling career.


The shocking case came to light when the girl was taken across state lines to terminate the pregnancy in Indiana on June 30, with President Biden highlighting it as proof of the evils of anti-abortion legislation.


However, despite Fuentes being 27, the person listed as getting the girl pregnant was described as a minor in the report her abortionist sent to authorities, according to Fox News Digital, which obtained a copy.


It was not immediately clear if Fuentes was identified in the document. Fox only said it showed how Bernard reported performing a non-surgical abortion for a 6-weeks-pregnant 10-year-old girl of Mexican ethnicity on June 30.
